Covid-19 Patient Screening Form

Instructions for filling out and sending the Covid-19 Patient Screening Form:

  1. Please download the Covid-19 Patient Screening form by clicking the button above to open the form in your browser.
  2. Then select the download icon (PDF).
  3. Please fill out, save and email to prior to your visit.
  1. Open PDF document. Click the Share button.
  2. After tapping the Share button, iOS will bring up a list of ways that you can share or save the file.
  3. Click Save to Files. Then choose a file location. The default setting is On my iPhone in the Downloads folder.
  4. Next open to your Files app. 
  5. Open saved Patient Screening Form. Fill out with name/age and checking the appropriate boxes.
  6. Then select the Share button again and email to

  3. Select Save As. Then if not already selected, tap Save as PDF on the drop-down list and then tap Save.
  4. Choose a location for your PDF, enter a new name (optional), and then tap Save.
  5. Next, open your Files app.
  6. Go to the Downloads folder. Open Patient Screening Form. Fill out with name/age and checking the appropriate boxes.

What to Expect for Your Next Dental Visit

Day of your dental appointment:

  • Please arrive 5 minutes prior to your scheduled appointment time.
  • On arrival please stay in your vehicle. Call the office 604-792-3324.
  • A dental team member will meet you at the office door to:
    • Complete the screening questions again.
    • Have your temperature taken.
    • Complete a form of acknowledging the risk of COVID-19.
  • When entering/exiting the office, please sanitize your hands.


  • ALL accompanying family members or parents MUST wear a mask if entering the office with a patient or child.
  • If possible, we are requesting parents/ family members wait outside.
  • We kindly ask you continue practicing social distancing measures.
  • Any patients showing signs of a fever or other symptoms of illness will be asked to reschedule their appointment.
